You may have seen the recent media coverage following a study published in JAMA Internal Medicine on 17 August 2026, reporting a higher proportion of deaths from cancers classified as radiation-related among U.S. pilots and flight attendants.

While these findings deserve attention, they should be interpreted with caution. The study is based on death certificates and occupational data. It does not include individual radiation doses, flight hours, routes, cumulative exposure or person-time. It therefore identifies an association, but cannot demonstrate that cosmic radiation caused the reported cancer mortality.

This cohort included 26,846 crew members, more than 750,000 person-years and over 510,000 individual dose records. Overall mortality and mortality from most cancers were lower than in the general German population,and no significant positive dose-response relationship was identified for most radiation-associated cancers. Some findings, particularly regarding brain cancer and melanoma, nevertheless remain relevant for continued scientific evaluation.
Another important aspect is the regulatory difference between the United States and Europe. Within the European Union, occupational exposure of aircrew is subject to specific radiation-protection requirements. Where exposure may exceed 1 mSv per year, operators are required to assess individual exposure, take radiation dose into account when organising work schedules and inform crew members about their individual exposure.
In the United States, aircrew are recognised as occupationally exposed to cosmic radiation, but there is currently no comparable federal system requiring routine individual dose monitoring and documentation for aircrew.
The new JAMA publication is therefore an important reminder to continue taking occupational cosmic radiation seriously. However, it does not demonstrate an increased overall mortality among aircrew, nor does it establish a causal relationship between cosmic radiation exposure and the reported cancer deaths.
